Our Orthopedic Care Coordinator assists you through the entire joint replacement process providing:

  • Training for your Coach -  the friend or family member helping you through your surgery
  • An informative pre-op Total Joint Class for patients and family
  • Detailed 70 page Patient Guidebook
  • Follow up by phone and in the clinic after surgery
  • Individual rehabilitation appointments to help you reach your goals

Your rehabilitation begins right in the hospital with individual and group exercises to speed you back to health.

 
 
 
 

Every R.E.A.C.H. patient is invited to attend a Reunion Luncheon after recovery where you can re-connect with your care team and build new relationships with people who’ve had similar experiences.

Pre-Operation Timeline 

  • Contact the Orthopedic Care Coordinator: (920) 746-0510

  • All necessary appointments will be scheduled for you and a list mailed to your home.

  • Recruit a friend or family member who will assist you through your recovery. We call that person your Coach. The Patient and Coach should attend the Total Joint Education Class.

  • See your primary care provider for a pre-operation physical within 30 days of surgery.

  • See your surgeon for a final preoperative exam and to have any final questions answered.

Post-Operation Timeline

  • Attend physical therapy evaluation prior to going home

  • Transition home the same day or next day after surgery

  • Attend outpatient rehabilitation the following day

  • Attend clinic follow-ups 2 weeks, 6 weeks, 3 months and 1 year out from your date of surgery

  • Most patients will go directly home after Total Joint Surgery. On occasion there are situations that may warrant a patient going to a care facility after Total Joint Surgery. If you believe you may go to a care facility after Total Joint Surgery you should check with your insurance company to determine if you have benefits for this type of care. If you do not have insurance coverage, then you will be billed for the care you receive there. See the Resources page for examples of facilities to contact.
